Necessary Documentation 

For those who are ELIGIBLE, you will need to prepare the following documentation in order to receive our services.

Income Documentation 

PROOF OF INCOME FOR ALL HOUSEHOLD MEMBERS IS REQUIRED

  • Last 4 paystubs (if paid weekly) Last 2 (if paid biweekly)​

  •  Self- Employment Income (Prior 3 mo. ledger sheets and two year tax return)

  • Last 4 unemployment paystubs or benefits award letter

  • Retirement / pensions benefits award(s) letter

  • social security benefits award(s) letter

  • SSI/SSD Workmen's compensation benefit Award(s) letter 

  • HEAP Award letter (regular benefits only)

  • Food stamp approval letter

  • Rental Income Receipt  (homeowners with Tenants)

ALSO REQUIRED- COPIES, NO ORIGINALS PLEASE

  • Social Security cards (for ALL household members)

  • Birth Certificates/ NYS Photo I.D. (either one)

  • Copy of front and back gas bill- MOST RECENT

  • Copy of front and back electric bill-MOST RECENT

  • 2yrs proof of gas usage (printout from gas supplier)​​

                         For National Fuel Customers - call 686-6123 ask for 2 yr. usage 

  • 2yrs proof of electric usage (printout from electric supplier)

                       For National Grid customers- call 1-800-642-4272 ask for 2 yr. usage

Deed/Mortgage statement

___Landlords name, address and phone (if you are the renter)

___County tax Bill (if you are the homeowner

** College registration (current) or High School letter if dependent is over 18 years old.

**Tenant Applications are also required even if not income eligible

Documents for Categorical Eligibility 

 For those are categorically eligible, make sure that you have the necessary' documents ready.
